Patient Demographic Entry

Patient demographics sheet contains all the basic demographic information about a patient. Each piece of information is important because correct and quality entry of such information will directly impact physician’s monthly revenue.

A good patient demographic form is the key to obtaining accurate information which is required for claim submission. Providing as much information as possible will reduce the number of rejections from the insurance companies. Most insurances will not process claims if any piece of information is missing from the demographics

Post to the service rendered to the patient, CNTI verifies all the Patient insurance and demographic details completely before updating the information in the Billing Software. On existing patients the team verifies the eligibility on cases when the patient returns for additional service. This department is responsible for entering all information received from the Physician’s office. It gets the information from the Client file, the Patient demographic file and the Charge ticket/ sheet. The process of capturing this information onto the billing software is called charge creation.

  • Address (physical and mailing address)
  • Home Telephone number
  • Employer name and address
  • Work Telephone number

2. Guarantor/Account details

  • Guarantor name
  • Date of Birth
  • Sex
  • Marital status
  • Social security number
  • Address (physical and mailing address)
  • Home Telephone number
  • Employer name and address
  • Work Telephone number

3. Insurance details

  • Insurance name and address
  • Insurance identification number
  • Group name/Group number
  • Validity of the policy
  • Name of the insured (subscriber)
  • Relationship of the insured to the patient/guarantor.
  • Financial Class (FC)
